What is nurse-led home care?

Nurse-led home care means a registered nurse can help the family understand health and day-to-day needs, ask the right clinical questions and clarify who should provide each part of care. It does not mean a nurse provides every visit.

What guides the conversation

Begin with the person, not a service list.

A useful plan considers what the person wants to keep doing, where help is needed, who is already involved and what the family can realistically manage.
